The Forms below may be printed, then filled out and returned to the Church Office.  However, these Forms are "fillable" PDF files.  You may open them by clicking on them below and instead of printing them you may fill them out on your computer and send them back to the Church office via email by clicking the "Submit" button (see below).

(After you've completed a Form in your computer, if you use internet/web based mail , i.e. gmail or yahoo, etc., when you click on the "Submit Form" button it will allow you to Save the document to your hard drive. Note the folder where you save it, then attach it from that folder to your email and send to:  office@stpeter-fallbrook.org.  Or simply print the completed form and drop it off at the church office.  If you use an email client like Outlook, Apple Mail, Eudora OSE, etc., you simply click the "Submit Form" button and it will open your mail client and insert the attachment for you.)
